However, no website, transmission method, storage system, or security measure can be guaranteed to be completely secure.

KMG Ventures LLC, through National Choice Health Plans, is not a healthcare provider and does not provide medical advice, diagnosis, or treatment.

HIPAA applies only in circumstances involving an organization that qualifies as a HIPAA-covered entity or business associate.

Information submitted directly through the National Choice Health Plans website is not automatically protected health information under HIPAA merely because it concerns health or medical matters.

Information may become subject to HIPAA or another privacy law after it is received by a benefit program, insurance carrier, plan administrator, healthcare provider, or other organization legally subject to those requirements.

12. Your Privacy Rights

Depending on your state of residence and applicable law, you may have the right to request:

  • Confirmation of whether we process your information

  • Access to personal information we maintain about you

  • Correction of inaccurate information

  • Deletion of information

  • A portable copy of certain information

  • A list of categories of information collected, used, or disclosed

We do not knowingly sell or share the personal information of consumers under 16 years of age.
